Acute interstitial nephritis

etiology

• Allergic: β-lactams, sulfa drugs, NSAIDs, PPIs
• Infection: pyelo, viral, legionella, TB, leptospirosis
• Infiltrative: sarcoid, lymphoma, leukemia
• Autoimmune: Sjögren’s, TINU, IgG4, SLE, ICIs

treatment

• Commonly drug-induced Acute interstitial nephritis
• If suspected, prompt removal of offending drug; ? early steroids w/in 7d of dx
• Rechallenging after suspected ICI-induced AIN may depend on oncology natural history
