Surgery for operable breast cancer after neoadjuvant chemotherapy

• Operable breast cancer preferred for neoadjuvant(牛阿珠) chemotherapy are
  • cT2N0 or cT2N1(only 1 or 2 nodes)
  • HER2-陽 breast cancer(preferable) or TNBC 三陰
  • Larger primary tumor relative to breast size in patients who desire BCS

• clips placement both at ↣ primary tumor and positive axillary lymph node(s),Surgery after Chemotherapy
  • BCS/Mastectomy
• Axillary management:
  • If cN0 both before and after C/T → SLNB
  • If cN1 become ycN0 after C/T, may consider SLNB + clipped node removal
  • If cN2 or 3 before C/T → ALND
