Allopurinol hypersensitivity syndrome

• 10-25% mortality;
• incidence ~5/1000.
• ↓ risk w/ starting dose 100 mg/d if eGFR >40 or 50 mg/d if eGFR ≤40.
• Titrate by 100 mg/d (eGFR >40) or 50 mg/d (eGFR ≤40) q2-5 wk until UrA <6 mg/dL (dose can be >300 mg/d even in CKD).
• A/w HLA-B5801, esp.
  • Han Chinese, Koreans, Thai;
• screen in these high-risk populations prior to initiating
