Primary Regimens of Pneumocystis pneumonia, PJP, Adult

• Adult Treatment; Patient able to take po meds:

• TMP-SMX DS, 2 tabs po q8h x 21 days + Prednisone 15-30 minutes before starting anti-PJP therapy: 40 mg po q12h x 5 days, then 40 mg q24h x 5 days, then 20 mg q24h x 11 days

• (Dapsone 100 mg po q24h + TMP 5 mg/kg po q8h) x 21 days + Prednisone as above

• Adult Treatment; Patient not able to take po meds*:*

• TMP-SMX 15-20 mg of TMP component/kg/day IV divided q6-8h x 21 days (can switch to oral therapy once able to absorb meds)  + Prednisone 15-30 minutes before starting anti-PJP therapy: 40 mg po q12h x 5 days, then 40 mg q24h x 5 days, then 20 mg q24h x 11 days

• Adult Prophylaxis: Primary prophylaxis/post-treatment of PJP suppression:

  • TMP-SMX-DS 1 tab po q24h or 3x/week OR TMP-SMX-SS 1 tab po q24h
  • Continue prophylaxis until CD4 count >200 for 3 months
