Alternative Regimens of Neutropenia-Febrile-High-Risk

• Initial therapy for fever: [Ceftazidime][18] 2 gm IV q8-12h; see comment for duration
• Enhanced Gram+ coverage: [Linezolid][19] 600 mg IV/po q12h or [Daptomycin][20] 6 mg/kg IV q24h (if not concerned for pneumonia)
