• Risk from major bleeding must be weighed against risk of valve thrombosis
• Not bleeding: if INR 5–10, withhold warfarin
• Bleeding: PCC (or FFP); reversal agent for DOACs; ± low-dose (1 mg) vit K IV if on VKA
