Definition-acute kidney injury (AKI)
(KDIGO 2012;2:1)
Stages in ICU correspond to ↑ hospital mortality and LOS (Crit Care Med 2009;37:2552)
Diagnostic criteria
- Increase in serum creatinine of ≥0.3 mg/dL within 48 hours or ≥50% within 7 days
- Urine output of <0.5 mL/kg/hour for >6 hours
Stage 1:
- Cr ≥0.3 mg/dL in 2d
- ↑ Cr ≥50%~100%
- UOP <0.5 mL/kg/h for ≥6h
Stage 2:
- ↑ Cr 2–3x baseline in 7d
- UOP <0.5 mL/kg/h ≥12h
Stage 3:
- ↑ 3x baseline in 7d,
- UOP <0.3 mL/kg/h for ≥24h,
- anuria ≥12h
- Cr >4
- Initiation of kidney replacement therapy
others
Cannot estimate GFR using Cr in setting of AKI or ∆’ing Cr (requires steady state) In patients <18 years, stage 3 AKI is also defined by KDIGO as a decrease in estimated glomerular filtration rate (eGFR) to <35 mL/min/1.73 m2.
Explain by ChatGPT
Acute kidney injury (AKI) is a sudden decrease in kidney function.
There are three stages of AKI:
Stage 1: A creatinine level increase of 0.3 mg/dL within 48 hours or an increase of 50% within 7 days.
Stage 2: A creatinine level increase of 2-3 times the baseline within 7 days.
Stage 3: A creatinine level increase of 3 times the baseline within 7 days.
