Autoimmune Etiologies of Liver Disease

Autoimmune hepatitis

•   Epidemiology: F:M = 4:1, associated with other autoimmune conditions

•   Clinical features: Variable presentation from asymptomatic to acute liver failure

•   Diagnosis:

-   Type 1: +ANA, +anti-smooth muscle (65%), +anti-actin, +anti-mitochondrial (with PBC overlap), anti-SLA/LP (10–30%), anti-dsDNA (25–35%), +pANCA

-   Type 2: +ALKM-1, +/−ALC-1

-   Liver biopsy

•   Treatment: Steroids +/−azathioprine. Indications: AST>10× ULN, AST>2× ULN and IgG level >2× ULN, bridging fibrosis, cirrhosis. Advanced disease may require liver transplantation.
