prophylaxis supportive medications during HSCT

  1. Conditioning regimen FluBu2
  • Fludarabine :50mg/dose/day x6 days, total 300 mg on day 5/7~5/12
  • Busulfan:3.2mg/kg* 2 day240 mg/day* 2 days on 5/95/10
  • Lorazepam 1 mg QID for seizure prophylaxis begins in the evening before the first busulfan dose, and continues until 1 day after busulfan infusion.
  1. GVHD prophylaxis [CsA+MMF+PTCy] -PTCy: 2600 mg (35 mg/kg /day, calculated by actual body weight) on 5/17 & 5/18. -CsA: 110 mg (~1.5 mg/kg) IV Q12H on 5/19 (D+5), check level on 5/23. -MMF: 750 mg PO three times daily (10 mg/kg TID) on 5/19 (D+5)

  2. Infection prophylaxis Bacteria : Ciprofloxacin 400 mg IV Q12H, started from D0 Fungal : Micafungin 50 mg IV QD on Day 0 and switch to Voriconazole on D+7. Voriconazole loading 6 mg/kg IV/PO Q12H for 2 doses followed by 4mg/kg IV/PO Q12H (5/7), check level on 5/13. CMV : Letermovir 240 mg QD, starting on D+5 HBV: Entecavir 0.5 mg qd HSV/VZV: valacyclovir 500mg bid PJP : TMP/SMX (80/400mg) : 1 SS tab PO QD, start during conditioning ,discontinue on neutropenia, then resume after engraftment for at least 6 months Oral hygiene: Chlorhexidine 0.12% mouthwash BID; Nystatin 4ml Q12H, may discontinue if micafungin is started
