• 當病人使用 STEP4,5 治療三到六個月，症狀仍無法緩解
  • 可稱 為: difficult-to-treat asthma
• 當盡一切所能包括 ≡ 吸入器使用技巧、改善 risk factor or contributory factor 等方式都無法改善症狀
  • 則稱 為: Severe asthma

1. 需評估病人 是: 否 為: type2 inflammation (Eosinophil count, allergen-driven, need maintainance OCS)
2. 檢杏 comorbidities/DDx
3. Social/psychological support
4. Multidisciplinary team care
5. Clinical trial

• 若病人評估 為: type 2 inflammation，在 ↣ 排除 AERD, ABPA 等原因且經濟或保險許可下，可 考慮下列幾種生物製劑
  1. Anti-IgE
  2. Anti-IL5/anti-IL5R
  3. Anti-ILAR
