Respiratory Alkalosis
↑pH, ↓PaCO2**, (Delayed compensation:** ↓HCO3–)
• Clinical features: Decreased cerebral blood flow: dizziness, anxiety; tetany (indistinguishable from hypocalcemia), arrhythmias if severe
• Etiology: Hyperventilation due to anxiety, pulmonary embolism, pneumonia, asthma, salicylates (early toxicity), pregnancy (due to progesterone), cirrhosis, sepsis, high altitude
- Acute: No renal compensation. Each 10 mmHg ↓PaCO2 → ↑0.08 pH, ↓2 mEq/L HCO3–
- Chronic: Renal compensation (less HCO3– absorb). Each 10 mmHg ↓PaCO2 → ↑0.03 pH, ↓5 mEq/L HCO3–
• Treatment: Correct underlying cause; inhaled mixture containing CO2 or breathe into a brown paper bag; do not need to correct in pregnancy
