classification of acute lymphoblastic leukemia (ALL)

Lymphoblastic neoplasms may present as

  • acute leukemia

    • (ALL)
    • w/ >20% BM blasts
  • lymphoblastic lymphoma

    • (LBL, more common in T-cell)
    • w/ mass lesion w/ <20% BM blast
  • Morphology:

    • no granules (granules seen in myeloid lineage)
  • Cytochemistry:

    • ⊕ terminal deoxynucleotidyl transferase (TdT)
      • in 95% of ALL, MPO negative

Immunophenotype

  • Precursor: CD34, TdT
  • B: CD19; variable CD10, CD22, CD79a
  • T: CD1a, CD2, cytoplasmic CD3, CD5, CD7
