Major Criteria
- Megakaryocyte proliferation & atypia w/ reticulin &/or collagen fibrosis, grade MF-2 or MF-3
- Does not meet WHO criteria for PV, CML, MDS, or other myeloid neoplasm
- Presence of clonal marker such as JAK2, CALR, or MPL Mt or no evidence of reactive fibrosis in absence of clonal marker
Minor Criteria
- Leukocytosis ≥11,000
- Elevated LDH
- Anemia
- Palpable splenomegaly
