Drug/Toxin-Induced Liver Injury
Acetaminophen
• Epidemiology: Most common cause of intrinsic drug-induced liver injury. Can occur due to accidental overdose, intentional overdose, co-ingestion with alcohol or other substances
• Diagnosis: Measure APAP level at 4 and 16 hours after ingestion. If timing unclear, draw immediately upon presentation and 4 hours later (levels may be negative up to 4 hours after ingestion)
• Treatment: N-acetylcysteine (NAC), consult poison control/toxicology
Other medications that cause liver injury
• Medications: Antibiotics (amoxicillin-clavulanate), antiepileptic drugs (phenytoin, valproate), antituberculosis drugs (isoniazid, rifampin), NSAIDs, azathioprine, Amanita mushroom poisoning
