• BM blast count <20%;
• requires absence of genetic abnormalities which would classify pts in another WHO category,
  • such as BCR–ABL, JAK-2, PDGFRα/β
• CMML : Chronic myelomonocytic leukemia
• RARS-T (RARS & thrombocytosis)
  • → RARS + plt count >450 K/μL; frequently JAK-2 V617F positive
