Major Criteria

• Megakaryocyte proliferation & atypia w/ reticulin &/or collagen fibrosis, grade MF-1 or less.
• In absence of fibrosis,
  • must have ↑ marrow cellularity,
  • granulocyte proliferation,
  • & ↓ erythropoiesis
• Does not meet WHO criteria for
  • PV,
  • CML,
  • MDS,
  • or other myeloid neoplasm
• Presence of clonal marker such as JAK2, CALR, or MPL Mt or no evidence of reactive fibrosis in absence of clonal marker

Minor Criteria

• Leukoerythroblastosis
• Leukocytosis ≥11,000
• Elevated LDH
• Anemia
• Palpable splenomegaly
