2014 Int’l Myeloma Working Group (IMWG) Myeloma-Defining Events (MDE): “SLiM”

• S = 60% percent, Clonal bone marrow plasma cells (BMPC) ≥60%
• Li = Light chains:
  • Involved/uninvolved serum free light chain ratio ≥ 100 AND TLDR: light chians 100x
  • involved serum-free light chain >10 mg/dL WARN:
• M = MRI: >1 focal lesion on whole body MRI or PET/CT

• C = Calcium ↑ (Ca >11 mg/dL or >1 mg/dL < nl);
• R = Renal insufficiency (CrCl; <40 mL/min or Cr > 2 mg/dL);
• A = Anemia (Hb < 10 g/dL or 2 g/dL < nl);
• B = Bone disease (lytic lesion/pathologic fx/sev. osteopenia). Also consider recurrent infxn, ↑ viscosity, amyloidosis
